The pandemic forced our Care Management teams to rethink how to safely and effectively function while still being a resource to our patients. A project was undertaken at Hillcrest Hospital to create a safe, effective and efficient way to utilize our Care Management teams' skills while ensuring the patients they were interacting with were of the highest need.
Hillcrest CM and CM Operations teams created a hybrid remote work model that utilizes initial screening criteria set to identify patients' needs and alerts CM teams of specific concerns that need to be addressed with those patients. In this session, speakers will describe the differences between the workflow prior to COVID-19 and how the pandemic altered the overall trajectory of the department. At the end of the session, you will understand the goals and objectives of the alternative workflow detailed in the presentation. Specifically, how we are continuing to fulfill the departmental goals related to patient care while working more efficiently and effectively in the CM role.
