Vivian Yu is currently a project manager for Cleveland Clinic Community Care (4C), which houses the primary care and population health functions of the organization. In this role, she is responsible for driving implementation and execution of operational and value-based initiatives, including access improvement, clinical services integration, and keepage. She also supports the west region primary care practices with Strengthening Teams in the Advanced Management of Populations (STAMP), the Clinic's vehicle for driving population health work. Prior to her position in 4C,Vivian served as a project manager in Executive Administration. Her work focused on enterprise-wide initiatives, special projects, and supporting the Office of the CEO.
Over the course of the past two years, the Cleveland Clinic's LGBTQ Employee Resource Group identified an opportunity for further LGBTQ education, awareness, and engagement. In consultation with key executive sponsors and nationally recognized content experts, the clinic designed and launched an enterprise-wide diversity and inclusion training series focused specifically to engage this underserved population. More than 500 caregivers have participated in the trainings since they went live in fall of 2018.
