Smarter Care for All

The HIMSS Smarter Care for All symposium will dive deep into social determinants of health (SDOH) and share how leading healthcare organizations are using digital technologies to transform the patient experience and deliver better outcomes for our most vulnerable populations.

The unique challenges facing these patients include poor access and utilization of healthcare services, lack of preventive health strategies, and ineffective chronic disease management.

Many of these challenges arise from social and behavioral factors, as well as an organization’s inadequate understanding of these patient factors. Patients are often more at-risk if they lack an advocate or transitions of care. The result is often poorly coordinated care that does not address the patient’s key goals for healing.
