Engaging patients and their caregivers is challenging for safety net organizations, given the unique factors related to health behavior, social determinants, and adherence to care recommendations.  Digital health technologies have emerged as potential patient-centered solutions to connect with and engage these vulnerable and at-risk populations, which are often uninsured or underinsured. 

In this session, attendees will learn effective technology solutions that can provide customized, interactive, and evidence supported strategies, including mobile messaging, patient portal, and real-time medication-use tracking solutions, as well as lessons learned from digital health initiatives in one of the country's largest safety net organization.
