In this session, we’ll hear how Providence St. Joseph Health (PSJH) created a Patient Engagement Center and developed a Consumer Engagement Platform to engage with the patients and improve care delivery. Patient navigation is increasingly complex for consumers with the rise of new care offerings such as virtual care, retail clinics, and finding the right specialty care and services. PSJH has addressed this challenge with a multi-channel approach including a technology/digital platform and human concierge services.
