Cleveland Clinic and MetroHealth patient and family centered care professionals, with key members of their PFAC's (Patient and Family Advisory Councils) will partner with attendees to message 'Moving Forward Together- Crayons, Conversations, and Community' by engaging messaging of relationship centered care. Four concepts of Patient and Family Centered Care: Dignity & Respect, Information Sharing, and Collaboration (integrating patients and families throughout health systems, to inform policies, facilities, and delivery of care) are at the core of discussion. Following a brief introduction to CC and MetroHealth's relationship engagement initiatives, speed-networking at roundtables facilitated by patients and PFAC leaders, will allow for: crayons, conversation, and community.
