In this session, Lisa Parks, quality improvement coordinator at Mid-Valley Behavioral Care Network (BCN), will discuss the challenges surrounding the behavioral health coordination in the ED. Lisa will discuss the actions BCN has taken to correct these issues, including the implementation of a care collaboration software that has enabled case managers and other providers to effectively track patient data and care team inputs. Using the insights from the collaboration tool, Mid-Valley has developed a targeted approach to connecting patients with their behavioral care specialists to prevent future readmission. This info has also helped Lisa follow-up with 99% of clinic patients within seven days of hospital discharge.
