Digitally Engaging One of the Nation's Most Underserved Populations

The asthmatic population that Saint Barnabas Hospital the Bronx serves is one of the most underserved population in the country, and little is known on whether new digital interventions might be adopted in this underserved population.

Last year, Saint Barnabas launched a project using a digital care prescribing platform to engage its asthmatic population outside the four walls of the health system where flare-ups typically happen. The goal was to equip patients with tools to assess the severity of their asthma, enhance education and self -efficacy in managing asthma for both English and Spanish speaking patients.

In this session, a project leader will describe in detail key takeaways, which include:

  • If designed in the correct way, extending care outside the hospital through digital care pathways can help strengthen the provider-patient relationship, rather than weaken it.
  • Engaging underserved patients in bulk through digital means is feasible. Saint Barnabas reached > 1,500 asthmatic patients in a week with a Digital Asthma Care Plan and saw a response rate of 19% from a “hard to reach” Bronx population. This surpassed expectations.
  • However, the finding that two-thirds of patients in the program had uncontrolled asthma leads to the question of finding capacity to see these patients in clinic. Or could telemedicine step in?
